MDS Coordinator - LVN part time
Vasona Creek Healthcare is seeking a full-time (LVN) MDS coordinator. You may be our next hire to join our team!The MDS Coordinator assumes full time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.QUALIFICATIONS/REQUIREMENTS:Current active license as a Licensed Vocational Nurse (LVN)Work Experience: At least 1 year experience in long term geriatric nursing care Experience in completing the Minimum Data Set is preferredLanguage Skills: Must be able to read, analyze, and interpret common scientific and technical information, and to be easily understood through verbal communication in the English language.Mathematical Skills: Must be able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimalsAbility to perform these operations using units of weight measurement, and volumeCommunication Skills: Must have exceptional communication and customer service skills, and be empatheticAbility to effectively communicate with patients, families, responsible parties, staff and outside resources and agenciesOther duties assigned Apply tot his job
