High-level Summary of Role & ResponsibilitiesÂ

This role will be involved with SDoH initiative planning, ongoing client relationship management (internally and externally) and operationalizing SDoH solutions, as well as leadership responsibilities including the management of Community Strategists. This role will also be responsible for ensuring that projects are aligned with the health plan’s overall health equity strategy developed by the Cultural and Healthcare Equity Director.Â

Job DutiesÂ
• Implement and scale the overall strategy to address Health Equity & SDoH for members and communities across stateÂ
• Operationalize Health Equity & SDoH product in local markets Â
• Hire, train, and manage field staff in assigned markets Â
• Establish and build key relationships with community-based organizations to execute Health Equity & SDoH strategyÂ
• Oversees and manages diverse portfolio of key community partnerships that align with national and states goals and objectivesÂ
• Identify opportunities to develop and pilot new Health Equity & SDoH initiativesÂ
• Gather input and maintain proactive communication across business units to coordinate Health Equity & SDoH function across the marketÂ
• Advance Health Equity by integrating SDoH solutions into population health, care management, quality, regulatory, and community outreach programs Â
• Partner with data analytics team to identify evaluation metrics and evaluate outcomes Â
• Oversee execution of community initiative funding supporting community gaps and member needs Â
• Organize community activation meetings to successfully execute key Health Equity & SDoH initiatives with community partners Â
• Create and support Business Development initiatives to align with Health Equity & SDoH strategyÂ
• Monitors, tracks, drives and functions as Market SME on all  SDoH data and deliverables Â
• Public Speaker at coalitions, conferences etc. to positively position the organizationÂ
• Oversee travel, expense report, budgetary parameters etc. for teamÂ
• Partner with Cultural and Healthcare Equity Director to align health equity initiativesÂ

Required QualificationsÂ
• 5 + years’ experience with Health Equity, population health, care coordination, and/or community-based programs Â
• 3 + years in a leadership roleÂ
• Ability to travel up to 25% of the time within the assigned region, to multiple areas with personal vehicle.Â
• Must possess reliable transportation and be willing and able to travel in-state up to 25% of the time. Mileage is reimbursed per our company expense reimbursement policy.Â

• Experience leading in a matrix environment Â
• Excellent interpersonal and strong communication skillsÂ
• Demonstrates good oral and written presentation skillsÂ
• Strong organizational skills, including the ability to prioritize, multi-task, and manage workload to meet specific timeframes and deadlinesÂ
• Prior Managed Care ExperienceÂ
• Master’s Degree or higher in health administration, social services, public health, business administration, psychology, or closely related fieldÂ

Educational Qualifications
• Bachelor’s Degree or higher in health administration, social services, public health, business administration, psychology, or closely related field or equivalent experience
• Â Masters preferredÂ
• Licensure or certification if applicable to degree Â

Pay Range

The typical pay range for this role is:

$60,300.00 - $132,600.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. Â The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. Â This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Â
