Position Overview As a LPN Home Care Nurse at Amedisys, you'll play a vital role in delivering skilled medical services to patients in their homes. You'll work collaboratively with our team of healthcare professionals to provide comprehensive care, educate patients and caregivers, and promote optimal health outcomes. This is a PRN (as-needed) position with immediate openings available, offering flexibility and autonomy in your work schedule. Key Responsibilities Verify Plan of Care: Review and verify the plan of care with the RN prior to each visit, ensuring that you're aware of the patient's needs and goals. Collaboration and Communication: Work closely with physicians, RNs, and other healthcare professionals to ensure that patient needs are being met, and notify them promptly of any changes in patient condition. Patient Education: Teach patients and caregivers self-care techniques, promoting independence and optimal health outcomes. Clinical Skills: Perform vital signs, medication reconciliation, and specialized procedures (e.g., foley catheter change) as ordered by physicians. Documentation: Complete timely and accurate documentation, ensuring that patient records are up-to-date and reflective of care provided. Qualifications and Requirements Clinical Experience: One year of clinical LPN experience in a home health or similar setting. Licensure and Certification: Current LPN license in the state(s) you're assigned to work, and current CPR certification. Driver's License and Transportation: Valid driver's license, reliable transportation, and liability insurance. Skills and Competencies: Strong communication and interpersonal skills, ability to work independently, and basic computer skills.
