Key Responsibilities:

Deliver professional nursing care to patients in accordance with established plans of care
Conduct comprehensive assessments, develop individualized plans of care, and provide skilled interventions to achieve realistic outcomes
Ensure compliance with federal and state regulations governing hospice care services
Provide education and support to patients, families, and caregivers
Collaborate with our clinical team to achieve exceptional patient outcomes and experiences
Maintain accurate and thorough documentation of patient care and progress

Requirements:

Graduate of an accredited school of nursing, Bachelor's degree preferred
Current Registered Nurse licensure (in good standing) in the state(s) of practice
One (1) year of experience as a nurse in hospice care or post-acute setting
Current CPR certification and valid driver's license
Strong clinical skills, excellent communication and interpersonal skills, and ability to work in a fast-paced environment
