We do this by embodying our WE CARE values.As a part time Medical Social Worker you will be an integral part of the care coordination team, working in collaboration with RN case management, the clinical team, and internal/external collaterals. As a member of our interdisciplinary team you will be responsible for performing psychosocial assessments, providing supportive counseling and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, and med/surg populations.Who We Are:

· A Master's degree in Social Work (MSW)
· Current Massachusetts Social Work licensure (LCSW/LICSW)
· A minimum of one year related experience in a hospital environment and/or with transferable skills that apply to a fast-paced and high-urgency setting
· Ability to triage crises and collaborate/coordinate with collateral agencies
· Experience with protective services, child welfare, long term care, crisis intervention, chronic illness, hospice care, substance use disorders and psychiatric illnesses a plus
