About the Role: Home Visitor

As a Home Visitor with SeniorsPlus, you will play a vital role in assessing the health, welfare, and unmet needs of clients, developing backup plans, and collaborating with Care Coordinators to ensure effective service delivery. This position offers the flexibility of working from a home office, with the opportunity to make a positive impact on the lives of disabled children, adults, and older adults participating in at-home, long-term care programs across Northern, Central, and Southern Maine.

Key Responsibilities:


Perform home visits to clients, ensuring timely and accurate documentation is communicated to Care Coordinators.
Assess clients' health, welfare, and unmet needs, developing backup plans and goals to support their independence.
Collaborate with Care Coordinators to ensure effective, efficient, and coordinated services.
Evaluate clients' plans of care to ensure continuity and effectiveness.
Positively impact the well-being of disabled children, adults, and older adults participating in at-home, long-term care programs.
Travel throughout assigned regions as needed, with mileage reimbursement provided.

Qualifications and Requirements

To be considered for this role, you will need:


A Bachelor's degree in Social Services, Human Services, or a Health-related field.
At least 1 year of customer service experience.
Strong computer and organizational skills.
Familiarity with the long-term care system is desirable but not required.
A valid driver's license and reliable transportation.


Schedule and Work Arrangements

This is a full-time position, with 8-hour shifts and occasional weekend availability. Travel is required throughout the assigned region, with mileage reimbursement provided.
