THIS ROLE IS A HYBRID ROLE THEREFORE MUST RESIDE IN MIAMI/DADE, FL, FOR CONSIDERATION.
• Monday through Friday
• 8:00 AM - 5:00 PM EST
• MUST RESIDE IN ONE OF THE FOLLOWING ZIPCODES 33182,33172, 33130, or 33165
• Pay Range $40.00 - $46.00
Summary:
Healthcare Consultant is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Healthcare Consultant will effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration.
Requirements:
• Bachelor's degree in Social Work or a related field
• At least 3 years of case management experience
• At least 3 years of prior authorization experience
• At least 1 year of long-term care experience
• Bilingual Spanish/English
• Proficiency in Microsoft Office such as Word, Outlook, Excel, and Teams
