Home Health Field LPN (PRN)
Position Summary
The Home Health Licensed Vocational Nurse (LVN) is responsible for providing one-on-one skilled nursing care and treatment to patients in their home. The LVN will provide comprehensive and patient specific care to promote their health and independence.
As a Home Health LVN, you will:
ā¢ Under the direction of the RN, assist in identifying the patient's physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
ā¢ Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
ā¢ Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
ā¢ Assure the continuity of care through delivery of quality patient care.
ā¢ Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
ā¢ Monitor assigned cases to ensure compliance with requirements of third party payor.
ā¢ Promote Agency philosophy and administrative policies to ensure quality of care.
Requirements
ā¢ Current LVN License
ā¢ Valid Driver's License
ā¢ Valid Auto Insurance
ā¢ CPR Certification
ā¢ HomeCare HomeBase experience preferred
