Provide virtual primary care assessments & chronic/acute care management
Create personalized treatment plans for diverse patient needs
Document thoroughly in EMR systems
Collaborate with a multidisciplinary team focused on holistic, culturally sensitive care
Engage in ongoing training to elevate telehealth best practices
Qualifications
Active FNP certification & state licensure (multi-state licenses preferred)
1-3 years of NP experience in primary care or similar
Strong dedication to improving health outcomes for diverse populations
Excellent communication & comfort working independently
Experience with telehealth platforms is a plus
