Job Overview We are seeking a highly skilled and compassionate Licensed Social Worker to join our team in a remote, full-time capacity. As a Licensed Social Worker at blithequark, you will play a vital role in partnering with registered nurse case managers to provide comprehensive support to our members. Your expertise will be instrumental in assessing, planning, and advocating for members with complex needs, including financial, transportation, safety, and housing challenges. You will work collaboratively with our clinical staff to empower members to make informed lifestyle and medical choices, and connect them with community resources that address their medical, financial, and behavioral health needs. Key Responsibilities Conduct telephonic assessments and develop personalized plans to address members' unique needs and goals Research and identify community resources, services, and programs that support members' financial, transportation, safety, and housing needs Collaborate with registered nurse case managers and other clinical staff to ensure comprehensive care coordination and support Empower members to make informed decisions about their health and well-being, and provide guidance on navigating complex health care systems Document member interactions, assessments, and plans in accordance with blithequark's policies and procedures Participate in ongoing training and professional development to enhance your skills and knowledge in medical social work and case management Essential Qualifications Active and unrestricted LSW, LMSW, LCSW, or LISW license in the state of residence Master's degree in Social Work or a related field Minimum of 2 years of experience in medical social work or case management Excellent communication, assessment, and problem-solving skills Ability to work independently and collaboratively as part of a remote team Strong knowledge of community resources, services, and programs that support diverse populations Preferred Qualifications Experience working with diverse populations, including those with complex medical and social needs Knowledge of health care systems, policies, and regulations Experience with electronic health records and care coordination software Bilingual or multilingual skills Certification in case management or a related field Career Growth and Development At blithequark, we are committed to supporting the growth and development of our colleagues. Compensation and Benefits We offer a competitive salary range of $46,597.20 - $101,228.40, depending on experience and location.
