About the Role We are seeking an experienced Medical Social Worker to join our team in Chattaroy, WA, on a per diem basis. As a Medical Social Worker at arenaflex, you will be responsible for assessing the social and emotional needs of our patients, developing personalized plans of care, and providing direction and supervision to Social Worker Assistants as needed. Your expertise will be essential in helping our patients navigate the complexities of their care, ensuring they receive the support and resources they need to achieve optimal health and well-being. Key Responsibilities Conduct comprehensive assessments of patients' social and emotional needs, taking into account their medical and nursing requirements, financial resources, and available community resources. Develop and implement personalized plans of care that address the unique needs and goals of each patient, in collaboration with our interdisciplinary care team. Provide emotional support, counseling, and education to patients and their families, helping them cope with the challenges of illness, injury, or disability. Identify and facilitate access to community resources, such as housing, transportation, and financial assistance, to support patients' recovery and independence. Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and communication. Participate in discharge planning, ensuring that patients have the necessary support and resources to transition safely to their homes or other care settings. Maintain accurate and timely documentation of patient interactions, progress, and outcomes, in accordance with arenaflex policies and procedures. Supervise and evaluate the performance of Social Worker Assistants, providing guidance and support to ensure the delivery of high-quality care. Essential Qualifications Masters or doctoral degree in Social Work from a Council on Social Work Education-accredited program. Current Social Worker licensure in the state of Washington, if required by state law or regulation. Minimum of one year of experience as a social worker in a healthcare setting, preferably in home health or hospice care. Valid driver's license, auto insurance, and reliable transportation. Proof of current CPR certification. Excellent oral and written communication, interpersonal, and organizational skills. Ability to work flexibly, including evenings, weekends, and holidays, as needed. Strong knowledge of community resources, discharge planning, and care coordination principles. Preferred Qualifications Experience working with electronic medical records and care management software. Knowledge of Medicare, Medicaid, and other healthcare regulations and standards. Certification in case management, care coordination, or a related field. Bilingual or multilingual skills, with the ability to communicate effectively with diverse patient populations. Experience with quality improvement initiatives and data-driven decision making.
