Key Responsibilities Deliver professional nursing care to patients in accordance with the established plan of care Complete comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters Formulate individualized plans of care that incorporate analysis of assessment data and modify and update as needed Provide skilled interventions aimed at achieving realistic outcomes within a specified time period Ensure thorough and accurate documentation of all clinical data, including admission through discharge, physician's orders, skilled nursing visits, and changes in conditions Perform on-call responsibilities as assigned and patient visits in an efficient manner, completing and submitting all documentation timely and per branch policy Determine if patients meet admission criteria and assist in the assignment of qualified personnel Assist physicians in developing plans of care and make necessary revisions based on patient conditions and needs Prepare clinical progress notes and review documentation on a continual basis to ensure compliance with hospice documentation standards Essential Qualifications Current RN license in the state of practice Minimum of 1 year of experience in hospice care or a related field Strong clinical skills and knowledge of hospice care principles and practices Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills Preferred Qualifications Experience with electronic medical records and documentation systems Knowledge of state and federal regulations governing hospice care services Certification in hospice and palliative care (CHPLN or CHPN) Bilingual or multilingual skills Career Growth Opportunities and Learning Benefits At Intrepid USA, we are committed to helping our team members grow and develop in their careers.
