Position Summary The Health Coach Consultant RN - Chronic Conditions Management role is a 100% remote opportunity, allowing you to work from home and reside in any state in the US. As a full-time position, you'll work 40 hours a week, with a schedule that includes: Monday-Friday: 9:00 am – 5:30 pm in your time zone 3 set days a week along with 2 set evening shifts per week from 12:30-9:00 pm in your time zone Weekends: rotating late Friday until 9 pm your time zone (typically every 4-6 weeks) and rotating Saturdays are required, per business needs (typically every 4-6 weeks) As a Health Coach Consultant, you'll utilize a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and promote healthy lifestyle choices among adults with chronic health conditions. Your expertise in acute Medical-Surgical Nursing and chronic condition education will be invaluable in this role. Key Responsibilities: Assess members through the use of clinical tools and information/data review to conduct comprehensive evaluations of needs and benefit plan eligibility for available integrated internal and external programs/services Utilize assessment techniques to determine members' level of health literacy, technology capabilities, and/or readiness to change Develop and implement personalized health coaching plans to promote healthy lifestyle choices and disease management Provide coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices Interpret and utilize clinical guidelines/criteria to positively impact members' health Monitor, evaluate, and document care plans in collaboration with members and/or attending physicians Identify and escalate quality of care issues through established channels Utilize influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes Essential Qualifications: Hold an unrestricted RN compact license in your state of residence and have the ability to be licensed in all non-compact states 3+ years of recent clinical experience in Adult Medical Surgical Nursing 3+ years of recent clinical experience providing health education about chronic health conditions to adults 3+ years of experience with documenting clinical notes in an electronic medical records (EMR/EHR) system 3+ years of proficiency with Microsoft Office applications: Word, Outlook, Excel, Teams, and PowerPoint 1+ year of experience with delivering health education coaching via telephone (1:1 and via webinar-based platform virtually to a group of attendees) Preferred Qualifications: Previous telephonic chronic condition coaching experience Virtual group coaching experience via webcam Managed care experience Experience in Epic EMR database functionality and documentation Excellent communication skills and comfort with public speaking Education: Associate's or Bachelor's degree in Nursing Career Growth Opportunities and Learning Benefits: At CVS Health, we're committed to the growth and development of our employees.
