As a social worker, you will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You will work with a diverse population spanning the entire lifecycle, utilizing telephonic and electronic communication to empower members to make positive lifestyle and medical choices. **Responsibilities** * Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Empower members to make positive lifestyle and medical choices through education and support * Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Utilize telephonic and electronic communication to engage with members and provide support * Maintain accurate and up-to-date records of member interactions and progress **Requirements** * Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence * 2+ years of experience in medical social work or case management * Masters degree in social work or related field **Preferred Qualifications** * Experience working with diverse populations, including children, adults, and seniors * Knowledge of community resources and services for medical, financial, and behavioral health * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Familiarity with electronic health records and case management software **Skills and Competencies** * Strong assessment and planning skills * Ability to prioritize and manage multiple cases and responsibilities * Excellent communication and interpersonal skills * Ability to work in a fast-paced environment and adapt to changing priorities * Strong problem-solving and critical thinking skills * Ability to maintain confidentiality and handle sensitive information **Career Growth Opportunities and Learning Benefits** At blithequark, we are committed to helping our employees grow and develop their careers.
