Our team of dedicated professionals is passionate about making a difference in the lives of those we serve, and we are seeking like-minded individuals to join our ranks. **Job Summary** As an Aftercare Case Manager, you will play a critical role in providing support and services to children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. Your primary responsibilities will include: * Providing crisis intervention and support to children and families as needed * Coordinating community and educational services to meet the needs of our clients * Maintaining accurate and timely documentation and reports * Conducting in-person meetings with children and families to support reunification and address concerns * Collaborating with public and private agencies to provide supportive services * Advocating for the needs and rights of our clients **Key Responsibilities** * Provide crisis intervention and support to children and families as needed * Coordinate community and educational services to meet the needs of our clients * Maintain accurate and timely documentation and reports * Conduct in-person meetings with children and families to support reunification and address concerns * Collaborate with public and private agencies to provide supportive services * Advocate for the needs and rights of our clients * Develop and implement individualized aftercare plans to support the needs of our clients * Provide education and support to children and families on topics such as parenting, life skills, and community resources * Participate in regular case management meetings to review client progress and adjust aftercare plans as needed **Essential Qualifications** * Bachelor's degree in human services or a related field * Valid driver's license * Ability to work independently and as part of a team * Strong communication and interpersonal skills * Ability to work with diverse populations, including children and families from various cultural and socioeconomic backgrounds * Ability to maintain confidentiality and handle sensitive information with discretion * Strong organizational and time management skills * Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities **Preferred Qualifications** * Experience working with children and families in a case management or social work setting * Knowledge of community resources and services available to support children and families * Experience with crisis intervention and de-escalation techniques * Bilingual or multilingual skills * Experience working with children and families who have experienced trauma or abuse **Skills and Competencies** * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong organizational and time management skills * Ability to maintain confidentiality and handle sensitive information with discretion * Strong problem-solving and critical thinking skills * Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities * Strong advocacy and support skills * Ability to work with diverse populations, including children and families from various cultural and socioeconomic backgrounds **Career Growth Opportunities and Learning Benefits** * Opportunities for professional growth and development through training and education * Collaborative and supportive work environment * Flexible scheduling and remote work options * Competitive hourly rate and benefits package * Opportunities for advancement and leadership development * Access to ongoing training and education to enhance skills and knowledge **Work Environment and Company Culture** * Collaborative and supportive work environment * Flexible scheduling and remote work options * Competitive hourly rate and benefits package * Opportunities for professional growth and development through training and education * Collaborative and supportive team environment * Opportunities for advancement and leadership development * Access to ongoing training and education to enhance skills and knowledge **Compensation, Perks, and Benefits** * Competitive hourly rate of $22.00 per hour * Opportunities for overtime and shift differential pay * $1,000 sign-on bonus * Flexible scheduling and remote work options * 20 days of personal leave in the first year of employment, increasing to 25 days after completion of year one * 12 paid holidays * Longevity bonus and tuition reimbursement * Opportunities for professional growth and development through training and education **How to Apply** If you are passionate about working with children and families to provide support and services that empower them to thrive, we invite you to apply for this exciting opportunity.
