Key Responsibilities
• Deliver direct patient care, including the administration of medications and monitoring of vital signs.
• Create and implement personalized care plans tailored to each resident's needs.
• Collaborate effectively with a multidisciplinary team, including physicians, nurses, and therapists.
• Uphold resident safety and comfort by adhering to infection control protocols and maintaining a hygienic environment.
• Educate residents and their families on health-related topics, fostering an understanding of care processes.
• Maintain accurate and timely documentation in resident medical records for continuity of care.
Required Skills
• Current and unencumbered LPN license in the State of South Dakota.
• Excellent communication and interpersonal skills.
• Empathetic and patient-centered approach to care.
• Strong team player with the ability to work collaboratively.
Qualifications
• Completion of an accredited LPN program.
• Experience in long-term care or a similar healthcare setting is preferred but not mandatory.
