The Licensed Clinical Social Worker (LCSW) is a mental health professional that independently provides psychosocial assessment and intervention to patients and families across a continuum of care.
What You Can Expect:
ā¢ Provides crisis intervention services to patients via video while they are in an ambulatory clinic or via phone in the community
ā¢ Provides consultation to physicians, healthcare professionals, or community partners in understanding psychiatric, behavioral, social, emotional, and cultural factors related to plan of care for the patient
ā¢ Consults with staff regarding clinical situations; assists inter-disciplinary team in dealing with complex, high-risk or difficult psychosocial situations
ā¢ Serves as a member of a multi-disciplinary team and is a liaison with community resources
ā¢ Provides community resource information and referrals
ā¢ May provide individual, group, or family therapy
ā¢ May participate in Third Party billing
ā¢ Other duties and responsibilities as assigned
Qualifications:
ā¢ Education: Masters degree
ā¢ Licenses/Certifications: Licensed Clinical Social Worker (LCSW) in Idaho

SLHS is dedicated to quickly addressing issues of suicidal ideation and other psychosocial stressors expressed by patients in the 300+ outpatient medical clinics across the health system. This role will provide on-demand support to these clinics across southern Idaho. This innovative position will provide the incumbent with a varied clinical experience of crisis intervention and supportive counseling via virtual functionality.
A successful candidate will be effective in conducting suicide risk assessments, intervening with risk mitigation strategies and familiar with crisis intervention practices. The candidate will proactively develop rapport with the outpatient clinic staff and be effective at fostering bidirectional communication. This position is also a remote patient management role and is the latest evolution of home-based monitoring-connecting patients with a care team tracking warning signs. The ideal candidate will be comfortable providing all services via a virtual platform. Patients have a daily connection with nurses and social workers who monitor vitals and look for behavioral signs such as depression or anxiety.
