Consultative Pharmacist - Remote

Specific tasks, responsibilities or competencies may be documented in the incumbent's performance objectives as outlined by the incumbent's immediate supervisor or manager. • Provide expert guidance on pain and symptom management tailored to individual patient needs and actively contribute to enhancing patient comfort and dignity during end-of-life care. • Collaborate directly with nurses and hospice staff to develop effective, cost-conscious medication care plans. • Educate healthcare professionals and caregivers on medication usage and adverse event prevention. • Accurately document clinical interventions and patient outcomes in electronic health systems. Ensure all encounter documentation is complete, clear, and ready for transcription/processing. • Maintain and update patient medication profiles and records in accordance with HIPAA and regulatory requirements. • Consistently meet or exceed expected productivity benchmarks. • Demonstrate exceptional communication and interpersonal skills in a remote or hybrid work environment. • Maintain a professional, empathetic tone in all patient and team interactions. • Engage with a multidisciplinary care team in a fast-paced call center environment. • Support team success through shared learning, collaboration, and knowledge sharing. • Participate in weekend rotations and contribute to team coverage as needed. Marginal or Additional Functions • Protects company's intellectual property and the privacy of customers' and patients' data • Performs other related duties as assigned or apparent. Supervisory and Managerial Responsibility • This position does not have supervisory or managerial responsibilities. Knowledge, Skills & Abilities Education, Licensure or Certification: • PharmD or B.S. in Pharmacy from an ACPE-accredited school of pharmacy • Active pharmacy license in home state (Pennsylvania or Tennessee) • Pharmacy licenses in other states or willingness to obtain additional licenses Work Experience or Related Experience: • Experience in hospice, palliative care, or oncology settings preferred • Previous experience working in a call center or remote clinical consultation environment preferred • Familiarity with pharmacy benefit management (PBM) services preferred Specialized Knowledge, Skills & Abilities: • Demonstrates competency in equianalgesic dosing, generic and therapeutic substitution, formulary management and prior authorization procedures • Demonstrates understanding, competency and adherence to all state and federal pharmacy practice rules and regulations • Strong clinical assessment and decision-making skills • Excellent written and verbal communication skills • Proficient typing and computer skills are paramount to employee satisfaction and success while fulfilling responsibilities • Excellent listening skills • Proven ability to manage multiple projects simultaneously while paying strict attention to detail • Ability to maintain confidentiality, and to exercise discretion and good judgment • Excellent interpersonal and customer service skills • Excellent organizational skills and attention to detail • Commitment to delivering high-quality, compassionate patient care • Proficient in Microsoft Office tools and electronic health record (EHR) systems Equipment: • Working knowledge of a PC, business and communications software (MS Office) and web-based tools is required Travel Requirements and Conditions • Limited travel may be required Work Environment, Conditions, and Demands • Work is performed in a remote or hybrid office environment. • Remote work Internet Policy Requirements: To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: • At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested • Satellite, cellular and microwave connection can be used only if approved by leadership • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information Physical Requirements and Demands Note: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions as described. • This position entails a high level of work performed at a computer terminal and telephone or headset equipment throughout the day • Regular eye-hand coordination and manual dexterity is required to operate office equipment • Ability to lift up to 30 lbs.
