Community Health Worker - Spanish Fluency Required

The Community Health Worker supports the operational and administrative functions of a multi-year program designed to shrink health disparities in the San Gabriel Valley.This is a grant funded position with a one-year contract. Responsibilities: Conduct outreach in the San Gabriel ValleySupport planning and coordination of a program activities and operations.Navigate community resources and share them with community members in need.Lead community listening sessions and share public health information on topics such as COVID-19.Coordinate and maintain applicable program schedules and agendas, which may include tracking deadlines, deliverables, communication with management and/or stakeholdersHelp deliver general welfare interventions to the communities of the San Gabriel ValleyUse software apps and tools to collect and report relevant data tracking community health metrics.Other duties as assigned by the Program and Area Managers Scope and Travel: Extensive travel by foot and by car within the San Gabriel Valley (Cities of Rosemead, El Monte, South El Monte, Altadena, Pasadena, Pomona, etc.), as well as travel to and from CORE’s Los Angeles Headquarters (Chinatown) Qualifications: Preference for candidate with bachelor’s degree in public health or health sciences, or at least two year’s equivalent experience in relevant fieldMust be fluent in Spanish Experience: Minimum 1 year experience in public health, healthcare settings, community outreach or related fieldsExperience in community outreach, conducting surveys, collecting data, and reporting dataExperience working with community-based organizations, local government agencies, volunteer groups in LA County (preferred)Knowledge of community resources in LA County Skills and Competencies: Ability to work independently and as part of a teamExcellent written and verbal communication skillsProficient in MS Office applicationsFlexible and adaptive to a frequently changing environmentExcellent organizational and problem-solving skillsClose attention to detail and a high level of accuracyBilingual in English and Spanish required (strong preference for reading/writing abilities in Spanish)Ability to walk 1-3 miles/day (reliable transportation required)Knowledgeable of HIPAA In accordance with CA law, the expected pay range for this CA position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
