Clinical Social Worker - Hybrid Opportunity

We are seeking a compassionate and experienced Clinical Social Worker to join our team at Mass General Brigham. As a hybrid opportunity, you will have the flexibility to work onsite 4 days a week and remotely 1 day a week, providing comprehensive care coordination and social work services to our patients.
Key Responsibilities:
Conduct comprehensive assessments and case management to identify high-risk psychosocial factors and develop personalized care plans
Provide patient navigation, health promotion, crisis management, and self-management education to empower patients and their families
Collaborate with interdisciplinary teams to ensure seamless care coordination and effective treatment plans
Offer short-term counseling, advanced care planning, and liaison services to community resources
Participate in team consultations and contribute to the development of best practices in social work

Requirements:
Current LICSW/LCSW in MA
Master's degree in social work
