Clinical - Senior LTSS Service Care Manager (RN)

Pay Range: $40 - $45/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
Completing assessments
Completing biannual
Telephonic outreaches
Care coordination
Follow up on request from members/LAR
Complete any assigned trainings.
Completing all required documentation
Submitting authorizations
Travel (home visit to complete assessments)
Maintaining at least 92% audit score
Providing member/LAR education
Productivity reports
Follow up with HEDIS measures
Assisting members/LARs with finding doctors, therapy agency, PCS agency, DME and etc
Describe the performance expectations/metrics for this individual and their team:
To complete work (listed above) with in turnaround time, as listed in work process and contract.
Maintain at least 92% on audits.
Tell me about what their first day looks like:
They will receive their log in information and spend time with IT setting up their equipment.
Completing trainings in University.
Then following days will be in training.
What previous job titles or background work well in this role?
They will have to have the RN license and pediatrics is a plus.

Bingual-Spanish
Education Requirement: RN
