Chronic Care Manager - LPN - REMOTE POSITION

Job Overview We are seeking a dedicated and compassionate Care Manager to join our healthcare team - fully REMOTE position. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. The Care Manager will work closely with patients, families, and healthcare providers to facilitate effective communication and promote optimal health outcomes. The Care Manager should have a strong interest and desire to engage with patients who need their assistance. Responsibilities • Conduct comprehensive assessments of patients' needs to develop individualized care plans. • Coordinate care across multiple settings including hospitals, ERs, and primary care facilities. • Collaborate with interdisciplinary teams to ensure continuity of care for patients, particularly in geriatrics and acute care situations. • Facilitate follow-up appointments to monitor patient progress. • Educate patients and families about treatment options, resources, and self-management strategies. • Advocate for patients’ needs within the healthcare system to enhance access to necessary services. • Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards Qualifications • LPN required • Recent Clinical Experience • Desire to help educate patients • Excellent communication skills with the ability to engage effectively • Proficient in case management software and electronic health records (EHR). • Commitment to providing compassionate patient-centered care.
