Certified Nurse Practitioner (Telehealth – RPM/CCM Services)

We provide Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services to seniors and individuals managing long-term health conditions. Our mission is to deliver high-quality care, improve outcomes, and prevent unnecessary ER visits through proactive, technology-enabled care. Position Overview We are seeking a licensed Nurse Practitioner (NP) to join our care team. This is a remote telehealth position where you will conduct virtual visits, health assessments, and ongoing RPM/CCM follow-ups with patients enrolled in our programs. You will play a key role in monitoring patients’ health, reviewing device data (blood pressure, glucose, weight, SpO₂), and helping optimize care plans in coordination with our physicians and care coordinators. Key Responsibilities • Conduct telehealth visits and initial health assessments for patients enrolled in RPM/CCM programs. • Review and interpret data from remote monitoring devices (BP cuffs, glucometers, scales, oximeters). • Provide ongoing patient education, support, and care plan adjustments as needed. • Collaborate with physicians, care coordinators, and nurses to optimize chronic care management. • Document all patient interactions, assessments, and care plans in the EHR. • Participate in care team meetings and contribute to quality improvement initiatives. Qualifications • Current, unrestricted Nurse Practitioner (NP) license in the U.S. • Board Certified (FNP, AGNP, or similar preferred). • Prior experience in primary care, chronic disease management, or telehealth strongly preferred. • Knowledge of RPM/CCM billing codes and workflows is a plus (training available). • Strong communication skills and ability to build rapport with patients remotely. • Comfortable using telehealth platforms, EHRs, and remote monitoring systems.
