Certified Nurse Practitioner (Telehealth β RPM/CCM Services)

We provide Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services to seniors and individuals managing long-term health conditions. Our mission is to deliver high-quality care, improve outcomes, and prevent unnecessary ER visits through proactive, technology-enabled care. Position Overview We are seeking a licensed Nurse Practitioner (NP) to join our care team. This is a remote telehealth position where you will conduct virtual visits, health assessments, and ongoing RPM/CCM follow-ups with patients enrolled in our programs. You will play a key role in monitoring patientsβ health, reviewing device data (blood pressure, glucose, weight, SpOβ), and helping optimize care plans in coordination with our physicians and care coordinators. Key Responsibilities β’ Conduct telehealth visits and initial health assessments for patients enrolled in RPM/CCM programs. β’ Review and interpret data from remote monitoring devices (BP cuffs, glucometers, scales, oximeters). β’ Provide ongoing patient education, support, and care plan adjustments as needed. β’ Collaborate with physicians, care coordinators, and nurses to optimize chronic care management. β’ Document all patient interactions, assessments, and care plans in the EHR. β’ Participate in care team meetings and contribute to quality improvement initiatives. Qualifications β’ Current, unrestricted Nurse Practitioner (NP) license in the U.S. β’ Board Certified (FNP, AGNP, or similar preferred). β’ Prior experience in primary care, chronic disease management, or telehealth strongly preferred. β’ Knowledge of RPM/CCM billing codes and workflows is a plus (training available). β’ Strong communication skills and ability to build rapport with patients remotely. β’ Comfortable using telehealth platforms, EHRs, and remote monitoring systems.
