We are seeking a Centralized Clinical Support LVN who is passionate about making a difference in our community. Our Centralized Clinical Support LVN participates in indirect patient care via telephonic and/or other electronic communications utilizing the navigation contact center software in a remote setting. Performs duties under direction of physician and/or nurse supervisor to assist in the provision of patient care. This position participates as part of the integrated health care delivery team and makes interpretations and exercises discretion in routine situations. The Centralized Clinical Support LVN will collaborate with employees at all levels within the organization, must be able to maintain professional working relationships, and provide excellent customer service. A Day in the Life of a Centralized Clinical Support LVN may look like this: • Perform care to meet the physical, psychological, psychosocial, emotional, and social needs of the patients and their families. • Coordinate and participate in providing indirect patient care via telephonic and/or other electronic communications • Manage telephonic and electronic communications, including patient health education and indirect care coordination, using LSCC’s navigation center software • Interpret reports, data, and medical information to determine the needs of the patient/family. • Document all patient interactions accurately in Electronic Health Record (EHR) system. • Perform prescription refills and provide medication management education. • Schedule appointments and complete prior authorizations/referrals as necessary • Communicate barriers, concerns, and patient health needs to appropriate nurse and/or provider • Collaborate with the interdisciplinary team to support patient care planning • Implement clinical interventions safely and appropriately, ensuring thorough documentation • Provide individualized patient coaching using health education tools and encourage patients to participate in improving and maintaining their health. • In collaboration with other members of the health team provide health education to patient/family or significant other (in compliance with HIPAA Standards) • Maintain patient care standards following established LSCC health education and information guidelines. • Participate in data collection, focus groups, TJC, PCMH, and other quality improvement initiatives. • Assume responsibility for his/her own professional development and for sharing knowledge with others. • Demonstrate understanding of national patient safety initiatives by strict compliance with all safety protocols and procedures. • Participate in all LSCC training as required. • Perform all other duties as assigned We ask our Centralized Clinical Support LVN to possess a minimum of: • Graduate of accredited nursing school • Valid State of Texas Licensed Vocational Nurse (LVN) license • Proficiency with MS Office Suite (Word, Excel, and Outlook) • Basic Life Support (BLS) certification from American Heart Association or American Red Cross The following experience/skills are preferred: • Previous experience in outpatient clinic • Previous work using electronic medical records (EMR) • Bilingual English/Spanish language skills Key Success Factors Some key factors that will make an individual successful in this role: • Time management • Problem solving skills • Detail oriented • Team player Benefits LSCC offers a competitive benefits package, including: • Competitive salary; • Medical, Dental, and Vision insurance; • LSCC paid Life insurance; • LSCC paid Short-Term and Long-Term Disability insurance; • Paid Time Off; and • 403b Employee Retirement Plan Apply tot his job
