Case Manager Virtual Intensive Case Management (2 Positions Open)

This position can work remotely, but the candidate must live in Maryland, DC or Virginia.

The Preferred Communities (PC) Intensive Case Management (ICM) program, funded by the Office of Refugee Resettlement (ORR) offers individualized case management to especially vulnerable clients. To address the growing demand for PC-ICM services, USCCB is launching a virtual PC-ICM program out of the Washington, DC office. The Virtual ICM (VICM) Case Manager will work in coordination with the PC-VICM team to deliver virtual intensive case management services and referrals to clients located across the United States.

The position must demonstrate willingness and ability to understand, respect and contribute to the USCCB mission and to fulfill job duties in accordance with its Catholic identity.

Responsibilities:

•Screen clients for VICM eligibility.

•Conduct client assessments at intake, and every 180 days until case closure via phone or video conferencing, using telephonic interpretation as needed.

•Collaborate with clients to develop individualized goals and service plans.

•Map community assets and make referrals to community partners in the client’s area.

•Assist clients in scheduling and navigating appointments.

•Help clients navigate healthcare and social service systems in their area.

•Input case notes into the USCCB database.

•Detail all client interactions in the case management tracking system promptly.

•Communicate client concerns to VICM Manager.

•Collaborate with community engagement and volunteer, Community Sponsorship & Engagement teams to develop partnerships and connect clients to relevant resources.

•Assess and make client referrals for additional services.

Level: Bachelor’s degree in social work, psychology, counseling or related field, can be substituted with relevant case management work experience

Number of Years Required: 2-4 years

The starting salary for this position is $61,025. This is the amount to be expected when starting in this position.

The upper limit for this position is $75,623. This is the amount to be expected after being in this role for an extended period of time.
