BHASO Care Coordination Supervisor - Remote in Colorado

The BHASO Care Coordination Supervisor will organize, collect, review and report behavioral health, social determinant of health and/or physical health information through Individual phone in-reach and outreach, while demonstrating multicultural responsiveness and effective communication skills with Medicaid members. This position follows established safety protocols in the community setting, as well as established preventive and disease management programs for health promotion and education. Deliver culturally responsive information regarding the availability of health and community resources that will reduce barriers to care. Connect Individuals to payers and services that the Individual is eligible for. Work collaboratively with the Regional Accountable Entities and the state. Lead care navigators and other team members involved in direct or indirect coordination of care.

Primary Responsibilities:
• Serves as a consultant to care coordination teams
• Provide direct care navigation, coordination and management for the BHASO
• Respect confidentiality and maintain confidences as described in the UHG Employee Handbook and acknowledged through signature by all employees. The ability to maintain confidentiality is a critical and essential component of this position
• Participate in Interdisciplinary care team meetings as indicated
• Serve as community liaison and maintain relationships with key individuals in the community and serve as an advocate by coordinating linkages or referrals to improve health, social, and environmental conditions for Individuals
• Coordinate and perform duties of communicating the mission and role of the organization to community based organizations
• Conduct Individual assessments
• Assess the changing needs and condition of the Individual and family and communicate this information to all involved Care Coordinators, community partners, providers and other appropriate individuals, according to department policies and procedures
• Document assessments, Individual/family response to care coordination interventions at the time of the encounter. Meet departmental standards and deadlines for timely completion of all required documentation and meet current agency productivity standards
• Educate and assist identified Individuals about behaviors that can enhance their health, successfully navigating the health system
• Facilitate connection and access to identified treatment needs
• Develop a plan of management associated with health care goals for each Individual addressing the diverse needs in a culturally responsive way
• Develop and maintain a report system for outcomes
• Communicate Individual issues requiring interventions to appropriate departments and providers
• Maintain confidentiality and uses only the minimum amount of protected health information (PHI) necessary to accomplish job related responsibilities. Maintain confidentiality of Individual information
• Participate in staff meetings, case conferences and in-services. Maintain familiarity with all policies and procedures that impact decisions and care
• This position requires travel Requires independent, reliable, flexible, and on-demand, transportation at the incumbent’s expense for travel between various locations and timely arrival and departure from various locations. If the employee chooses to satisfy this requirement by driving a vehicle, the employee must meet the requirements for Colorado licensure and company requirements for liability insurance coverage

Required Qualifications:
• Possess one of the following:
• Master’s degree in a behavioral health field such as Social Work, Counseling, Pastoral Counseling, Psychology or similar field, and 2+ years working in a clinical setting and/or a related administrative setting
• A bachelor’s degree in any field, with 4+ years of experience working in a clinical setting
• Knowledge of health education, motivational strategies, and an empathetic manner working with underserved populations
• Driver’s License and access to a reliable transportation and ability to travel up to 25%Resident of Colorado

Preferred Qualifications:
• 2+ years of progressive related experience working with diverse populations, community or faith-based organizations
• Experience with Colorado’s Behavioral Health Administration
• Health care setting experience
• Knowledge of health care business
• Knowledge of local area
• Bilingual in English and/or Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
