- Identify high-risk patients and prioritize needs for interventions
- Develop and implement treatment plans and interventions
- Coordinate care with appropriate resources and conduct home visits
- Monitor completeness, accuracy, and timeliness of documentation
What is Required (Qualifications):
- Bachelor's Degree in Social Sciences (Required)
- Minimum of 2 years of related work experience (Required)
- Certification(s) and License(s): N/A
- Skills: Assessment And Diagnosis, Patients Rights, Teamwork, Time Management
How to Stand Out (Preferred Qualifications):
- Preferred experience working in the substance abuse field or with individuals with substance use disorder
