A progressive telehealth group is hiring a Nurse Practitioner (NP) to support patients through Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs. This role allows providers to work remotely while helping patients manage chronic conditions and reduce hospitalizations. Key Responsibilities: • Conduct virtual assessments, develop care plans, and manage chronic conditions • Monitor RPM data (BP, glucose, O2, vitals) and intervene on abnormal readings • Educate patients on self-care, lifestyle changes, and adherence • Collaborate with physicians, pharmacists, and interdisciplinary teams • Ensure compliance with CMS billing guidelines and HIPAA regulations Qualifications: • Active NP license (state-specific) with prescriptive authority • National board certification (FNP, AGNP, or equivalent) • New graduates welcome (chronic disease management experience a plus) • Bilingual English/Spanish strongly preferred • EMR/EHR and telehealth proficiency Benefits & Compensation: • Hourly Pay: $20 – $50 per hour • Medical insurance • PTO and parental leave • Flexible, remote schedule • Growth opportunities in population health and telemedicine Company DescriptionNPHire is the go-to job board for Nurse Practitioners.
