All Jobs > Health Coach (Diabetes & Lifestyle) Wellworks Apply Health Coach (Diabetes & Lifestyle) Fully Remote • Remote Worker - N/A Apply Description The Diabetes & Lifestyle Health Coach is responsible for supporting members in meeting their health goals and the objectives set forth in their company’s Wellness program through virtual health coaching sessions and diabetes education. This position will serve as a health coach on both the Diabetes and Lifestyle coaching teams, and the allocation of hours to each team will depend on factors such as time of year and client need. Duties and responsibilities General Overview Help members create and meet health goals and objectives. Appropriately adapt coaching and education methods to accommodate the unique physical, psychological, cultural, and age-specific needs of each member. Demonstrate established customer service skills. Understand the wellness programs of each company and keep members on track to meet program goals. Follow established program policies and procedures to support consistent coaching practices and program quality. Lifestyle Serve as a health coach for members looking to create or maintain health behavior changes in areas of well-being such as sleep, movement, nutrition, stress management, weight management, tobacco cessation, and improving biometric values. Diabetes Provide health coaching and diabetes education to members enrolled in the Diabetes Care Program, supporting individuals with prediabetes, type 1, and type 2 diabetes in effective self-management. Review member-reported health data, including glucose readings, to identify patterns and trends and provide education and coaching to support effective self-management. Support members in understanding their health data and encourage appropriate follow-up with their personal healthcare providers when concerns arise. Provide education and coaching aligned with evidence-based diabetes and lifestyle management guidelines, including ADA Standards of Medical Care in Diabetes, AHA Hypertension Clinical Guidelines, and CDC Diabetes Prevention Program (DPP) lifestyle recommendations. Requirements Bachelor’s degree in Nursing, Nutrition, Health Promotion, Public Health, Health Education, Exercise Science, or related field (required) National Board Certified Health and Wellness Coach (required) Registered Nurse or Registered Dietitian Nutritionist (preferred) Certified Diabetes Care and Education Specialist (preferred) CHES/MCHES (preferred) Strong understanding of HIPAA compliance 2+ years with direct experience in diabetes care and education 2+ years of experience in direct health education or health coaching with individuals for disease prevention, health promotion, and behavior change Experience working with digital health technologies, coaching platforms, and Microsoft 365 (Word, Excel, Outlook, Teams) Knowledge of disease prevention and management, exercise principles, nutrition, and weight management Ability to work both independently and as part of a team with enthusiasm, initiative, and creativity Flexible and able to balance the demands of Diabetes coaching and Lifestyle coaching Bilingual in English and Spanish is a plus Why work with us? Local candidates can have the option of hybrid work in office and remote if desired. Because you will be handling confidential client information, you must work from your designated home office. You cannot work in a public location. Computer equipment and VPN access are provided. Employee must provide their own internet access. This position is full-time and works Monday through Friday 7 hours per day with a 1-hour lunch break. Schedule is flexible depending on the time zone of employee, but must be between 8am-8pm EST.
