ABOUT THE POSITION (Remote)The Clinical Pharmacist plays a pivotal role in the healthcare team, providing essential pharmacy services that contribute to optimal patient outcomes. Reporting directly to the Specialty Pharmacy Director, this position involves evaluating pharmacotherapy to enhance treatment efficiency, effectiveness, and cost-effectiveness. Key responsibilities include monitoring drug therapy, ensuring compliance with regulatory standards, and delivering patient education to ensure safe and effective medication use.This role blends direct patient care with interdisciplinary collaboration, actively participating in quality improvement initiatives and leadership opportunities. The Clinical Pharmacist is responsible for reviewing and optimizing medication regimens, addressing drug-related issues, and supporting transitions of care. By participating in safety, quality, and cost-savings initiatives, the Clinical Pharmacist contributes to institutional goals, all while maintaining a strong focus on patient-centered care and clinical excellence. Principal Duties and Responsibilities Oversee therapy management in accordance with organization policies and protocols, identify and address medication errors and adverse reactionsConsistently demonstrate the ability to respond to changing situations in a flexible manner to meet current needs assigned by the Specialty Pharmacy Director or hospital leadershipComplete appropriate documentation promptly, following Client policies and proceduresContribute to the achievement of Quality Improvement and participates in QI activities Ensure adherence to established standards, client policies, and Visante procedures. Report and resolve safety issues and incidents promptlyPerform other duties as assigned by the Specialty Pharmacy Director, Visante Leadership team, or designeeProvide appropriate medication therapy and address patient concerns confidentiallySupport medication use processes to ensure safety, efficacy, and compliance with guidelinesIdentify and resolve potential drug-related issues to optimize patient outcomesMaintain accurate records of clinical interventions, adverse drug events, and quality improvement activitiesParticipate in initiatives to enhance medication safety, operational efficiency, and cost-effectivenessWork with interdisciplinary teams to support effective patient care and program developmentRespond to evolving departmental needs and contribute to various projects or initiatives as assigned
Requirements
EducationRequired: Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy. Preferred: Successful completion of an ASHP accredited PGY1 or specialty residency. ExperienceRequired: NonePreferred: 3 years of retail or specialty pharmacy. Licensure/Certification Required: Current Pharmacist license as granted by the appropriate state licensing authority obtained within 2 months of hire date. American Heart Association BLS certification obtained within 6 months of hire date.Preferred: Board of Pharmacy Specialties certification. Skills and Abilities Strong understanding of pathophysiology and pharmacotherapeuticsAbility to interpret and apply regulatory standardsDemonstration of good judgement, multi-tasking, and meeting deadlines with a sense of urgency and being able to prioritize competing demandsStrong client relationship, interpersonal, and team skillsExcellent presentation skills with the ability to make effective presentations to all levels of managementProven ability to diagnose and resolve issues, demonstrating strong analytical and creative skillsAbility to make sound, timely decisions based on analysis, experience, and judgementClear and concise verbal and written communication skills and the ability to advise clients professionally and positivelyCompensation and Benefits: We offer competitive salary and benefits for this full-time salaried role.
