Position Details Primary Department or Practice: ----- Ready to shape the future of healthcare? UHS is looking for a Virtual Health Provider - Nurse Practitioner (NP) to join our cutting-edge Hospital Care at Home program. In this role, you will provide essential care to a high-acuity patient population, including those recently discharged from the hospital, ED, and walk-in clinics. Your expertise will be crucial in preventing readmissions and empowering patients to heal in the comfort of their homes. This is a chance to step into a fast-growing and vital field. You will provide care to patients in their homes using telemedicine as part of a team-based model of care which will at times include community paramedics. You can either work remotely or in a hybrid model. You must be board certified/eligible and licensed to practice in New York State. Must have completed an accredited Family Medicine NP Program.
