Job Description

As a Remote Nursing Coordinator, you will play a crucial role in managing and coordinating the care needs of our patients. You will be responsible for ensuring that patient care plans are executed efficiently and effectively, while also providing support to our healthcare team. This position requires a dedicated nursing professional who is passionate about making a difference in patients’ lives by providing high-quality care from a remote setting.

Your primary responsibilities will include evaluating patient care needs, coordinating with healthcare providers, and ensuring compliance with healthcare regulations. You will leverage your nursing expertise to facilitate communication between patients, their families, and healthcare providers, ensuring all parties are informed and engaged in the care process.

Requirements

1. Valid Registered Nurse (RN) license in the United States.

2. Minimum of 2 years of nursing experience, preferably in a care coordination or similar role.

3. Strong communication and interpersonal skills.

4. Proficiency in using electronic health records and other healthcare software.

5. Ability to work independently in a remote setting.

6. Knowledge of healthcare regulations and compliance standards.

7. Strong organizational and time-management skills.

Responsibilities

1. Evaluate and assess patient care needs and develop individualized care plans.

2. Coordinate with healthcare providers to ensure seamless patient care.

3. Monitor patient progress and make necessary adjustments to care plans.

4. Communicate effectively with patients, families, and healthcare teams.

5. Maintain accurate and up-to-date patient records.

6. Identify and address any potential issues in patient care promptly.

7. Provide support and guidance to other healthcare team members.

Educational Qualifications
