We are seeking qualified and compassionate professionals to join our team as CCM Monthly Callers. Open Positions: • California Licensed CNA (Certified Nursing Assistant) • California Licensed LVN (Licensed Vocational Nurse) Position Details: • Part-Time, Remote • Flexible Schedule • Opportunities for professional growth within an expanding organization • Rate: $21 per hour Role Overview: As part of our CCM team, you will conduct monthly outreach calls to patients with chronic health conditions such as diabetes, hypertension, and COPD. The goal is to support ongoing care, encourage adherence to treatment plans, and identify any emerging health concerns.
