Job Summary We are seeking a results-driven and compassionate Transition of Care Coordinator to join our team. As a Transition of Care Coordinator, you will be responsible for facilitating smooth transitions of care for our members, utilizing your clinical expertise to drive positive health outcomes, and working closely with healthcare providers to ensure seamless coordination of care. Key Responsibilities Conduct comprehensive assessments of members' needs and develop individualized care plans to ensure seamless transitions of care. Collaborate with healthcare providers, including hospitals, physicians, and other healthcare professionals, to ensure coordination of care and services. Utilize clinical judgment and decision-making skills to identify and address complex clinical issues and develop strategies to mitigate risk. Provide education and support to members and their caregivers on self-management techniques, medication management, and follow-up care. Conduct regular follow-up visits and communicate with members and their caregivers to ensure they are receiving necessary care and services. Apply clinical expertise to identify and address gaps in care, and develop strategies to improve health outcomes. Utilize case management tools and techniques to coordinate care and services, and to ensure that members receive necessary support and resources. Participate in interdisciplinary care teams to ensure comprehensive care coordination and to address complex clinical issues. Maintain accurate and up-to-date records of member interactions, care plans, and services provided. Requirements Bachelor's degree in a related field (e.g., nursing, social work, or counseling). Oklahoma licensure as a Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Behavioral Specialist (LBS), or Licensed Clinical Social Worker (LCSW). 3+ years of direct clinical practice experience post-graduate degree, preferably in a hospital or alternative care setting. 2+ years of experience with computer systems, including bolthires Office Suite applications (e.g., Outlook, Word, Excel, SharePoint). Strong transportation and willingness to travel in-state up to 50% of the time (mileage reimbursement provided). Preferred Qualifications Experience with crisis intervention and management. Managed care/usage review experience. Case management and discharge planning experience. Experience providing care to Native American or other underserved populations.
