We're seeking a dedicated and empathetic Registered Nurse (RN) Case Manager to deliver exceptional patient care in a home health setting. As a valued member of our team, you will play a vital role in coordinating and integrating healthcare services for our patients, ensuring seamless transitions and optimal outcomes.
About the Role:
Full-time, day shift position with a competitive salary range of $34.00 - $51.00 per hour
40 hours per week, with occasional weekends and evenings required
Remote work arrangement with travel to patient homes in the Hays, KS area
Opportunity to work with a diverse patient population, including neonatal, pediatric, adolescent, adult, and geriatric patients
Key Responsibilities:
Integrate healthcare services for patients from pre-admission to post-discharge, ensuring coordinated and sequenced care
Evaluate patient outcomes using established clinical, financial, and utilization data
Utilize independent nursing judgment to make informed decisions and develop personalized care plans
Communicate effectively with patients, families, and interdisciplinary teams to promote collaboration and patient-centered care
Document patient information accurately and efficiently, using electronic medical records
Requirements:
Bachelor's degree in nursing preferred, with a minimum of one to two years of professional nursing experience in a clinic or home health setting
Current, unencumbered RN license with the State Board of Nursing, or multistate licensure if in a Nurse Licensure Compact (NLC) state
Strong knowledge of specialty area, with in-depth understanding of clinical pathways, guidelines, and standards of care
Excellent communication, problem-solving, and critical thinking skills, with ability to work independently and as part of a team
