Key Responsibilities:
Collaborate with the MHSU Care Coordination Department in daily activities
Connect grant program recipients to housing resources through participant engagement
Outreach to identify and support households experiencing homelessness
Maintain a required caseload and provide trauma-informed services
Develop relationships with community stakeholders to enhance service provision
Requirements:
Bachelor's Degree in Human Services or a related field
Minimum of two (2) years of post-degree experience in Mental Health and Substance Use (MHSU) services, or four (4) years in a non-Human Service field
Strong customer service and time management skills
Technological proficiency and attention to detail
Ability to travel as needed
Preferred Qualifications:
Experience working with HUD or Emergency Solutions Grant funded programs
Familiarity with Homeless Management Information System (HMIS)
Knowledge of community resources and housing regulations
