Position Overview We are seeking Internal Medicine and Family Medicine physicians (MD/DO) to support AWE (Annual Wellness Exams) and TCM (Transitional Care Management) services via telemedicine. This role is ideal for providers looking for flexible, remote work while maintaining continuity and quality of care for patients. Licensure Requirements • Active, unrestricted medical license(s) in assigned states • Multi-state licensure preferred (additional licenses may increase compensation) Responsibilities • Conduct Annual Wellness Exams (AWE) via telemedicine • Perform Transitional Care Management (TCM) visits following hospital or facility discharge • Review medical history, medications, and care plans • Coordinate follow-up care and ensure appropriate documentation • Provide patient education and preventive care guidance • Accurately document encounters within the EMR • Comply with CMS guidelines and state-specific regulations Experience • Proven experience working with EHR systems such as Epic or similar EMR platforms in outpatient or hospital environments. • Demonstrated proficiency in medical imaging interpretation or ordering diagnostic radiology procedures like CT scans or nuclear medicine studies is preferred. • Prior telehealth experience is highly desirable, with familiarity using telemedicine platforms for remote patient engagement. • Strong knowledge of internal medicine or family medicine principles with the ability to manage complex cases involving multiple comorbidities. • Experience providing care in diverse clinical settings including inpatient hospital medicine, urgent care, or nursing home environments. • Familiarity with advanced procedures such as suturing, IV infusion management, airway management, or basic life support (BLS) certification is advantageous. • Ability to perform comprehensive physical examinations virtually while maintaining excellent patient service and communication skills. • Knowledge of medical coding (CPT/ICD) for billing purposes and understanding of documentation standards aligned with NCQA or hospital accreditation requirements is beneficial.
