About the Role

The Telehealth RN Clinical Coordinator is a critical front-line clinical role responsible for assessing patient concerns and guiding them to the appropriate level of care through virtual platforms. Serving as a primary access point for patients, the RN Clinical Coordinator ensures safe, timely, and patient-centered triage while supporting continuity of care across the Dimer Health clinical team.

This role blends strong clinical judgment with excellent communication and care coordination skills in a fully remote, virtual-first environment.

KEY RESPONSIBILITIES

Clinical Triage & Decision-Making
• Conduct comprehensive clinical triage via phone, video, and secure patient messaging
• Assess patient symptoms, identify red flags, and determine urgency and appropriate disposition, including:
• Immediate referral to the Emergency Department (ED)
• Same-day or urgent virtual visit with an APP or physician
• Non-urgent issues appropriate for monitoring or near term follow-up
• Utilize Dimer Health’s problem-specific protocols within RN scope of practice
• Escalate cases to providers appropriately and collaborate in real time when clinical complexity warrants

Patient Communication & Education
• Provide clear patient education, self-care guidance, and follow-up instructions
• Communicate calmly and empathetically with patients experiencing acute, chronic, or new symptoms or concerns
• Support medication refill requests and address other clinical inbox matters within scope

Care Coordination & Team Collaboration
• Serve as a clinical liaison between Administrative and Clinical Teams to support timely care delivery
• Assist with care coordination in partnership with the Case Manager to support continuity of care
• Support timely scheduling and patient “conversion” to appropriate visits when indicated

Clinical Inbox, Documentation & Operations
• Review and respond to patient messages within defined service-level expectations
• Prepare and distribute work notes, school notes, and other post-visit documentation as appropriate
• Document all patient interactions accurately and thoroughly in the electronic medical record (EMR)
• Perform administrative and operational support tasks as needed to ensure efficient care delivery

Compliance & Privacy
• Maintain patient privacy and confidentiality in accordance with HIPAA and telehealth regulations
• Adhere to clinical, operational, and quality standards in a remote care environment

QUALIFICATIONS

Required
• Active, unrestricted Registered Nurse (RN) license in both New York and New Jersey
• Minimum 1+ year RN experience, preferably in triage, urgent care, emergency department, or telehealth settings
• Strong clinical assessment, critical thinking, and decision-making skills
• Utilizes crystal-clear verbal and written communication skills to accurately inform and guide patients through telephonic care
• Ability to assess acuity and risk without in-person physical examination
• Comfort working in a fast-paced, virtual-first care model with high message volume
• Proficiency with EMR systems and digital health technology
• Reliable high-speed internet connection and a private, secure home workspace that is HIPAA compliant
• Flexibility to work some evenings, weekends, and holidays as part of a rotating schedule

Preferred
• Prior telehealth or RN triage experience
• Experience using Athena EMR
• Bilingual (English/Spanish or other languages a plus)
• Multi-state compact licensure

• The displayed salary range reflects the minimum and maximum target for new hire salaries for the position across all locations. Within the range, individual pay is determined by factors including job-related skills, experience, relevant education or training. We typically make offers near the midpoint of the posted salary range.

Pay: $78,000.00 - $88,000.00 per year
