If you’re passionate about improving healthcare for polychronic seniors and making a real impact, we’d love for you to join us!About This RoleBelle is hiring remote Physician Assistants, Clinical Nurse Specialists or Nurse Practitioners that are licensed in the state of Georgia. The primary purpose of the role is to conduct telehealth Annual Wellness Assessments and establish Chronic Care Management (CCM) care plans for qualifying, polychronic members. This role is 100% remote, allowing you to utilize your healthcare expertise while maintaining a flexible working environment. In this position, you will perform wellness evaluations, gather comprehensive patient histories, and collaborate with our team to enhance patients’ overall health. Anticipated availability is 9am-6pm EST, Monday-Friday. No major holidays, on-call or weekends.ResponsibilitiesConduct telephonic wellness assessment visits.Identify patient needs, provide health education, and offer lifestyle recommendations to support preventive health measures.Create patient care plans.Efficiently document patient interactions and maintain accurate records in compliance with company policies and healthcare regulations.Collaborate with the care team to ensure patients receive appropriate referrals and care. Identify and enroll patients who would benefit from CCMReview ongoing CCM patient care plans.Experience, Skills, & PersonalityAppropriate CNS, NP, PA licensure/certification in Georgia.Willingness to obtain licensure/certification in additional states. Compliant with requirements for CEUs and licensure in all states where a license is heldExperience in telehealth, wellness visits, and preventative care is required.Excellent communication and interpersonal skills, especially in a telehealth setting.Strong organizational skills and attention to detail for effective patient documentation.Comfortable utilizing and managing electronic health record (EHR) systems, medical software/technologies to ensure accurate documentation, and data security.
