Job Summary
As a Maternity RN Case Manager, you will play a vital role in coordinating care for high-risk maternity members, providing evidence-based education and support to promote healthy pregnancy, delivery, and postpartum outcomes. Key Responsibilities
Provide trimester-specific and postpartum education to promote healthy pregnancy, delivery, and postpartum outcomes
Coordinate care for high-risk maternity members and proactively engage them in managing their own health and healthcare
Conduct outbound calls to assess members' current health status, identify gaps or barriers in treatment plans, and educate them on disease processes
Collaborate with Medical Directors, providers, and internal and external partners to ensure comprehensive care
Document and track findings, and create a positive experience and relationship with members and the team
Requirements
Current and unrestricted Missouri RN license
3+ years of OB or NICU clinical nursing experience
2+ years of experience working in Medicaid and in a hospital setting, acute care, or as a telephonic Case Manager for an insurance company
Proficiency with Microsoft Office Suite and electronic clinical records
Willingness to travel up to 50% of time in the assigned region
Preferred Qualifications
Certified Case Manager (CCM) or Certified Lactation Consultant (CLC)
Experience working with high-risk maternity populations and in managed care
