Responsibilities
• Conduct assessments and provide counseling to patients and families regarding coping and management related to Palliative and End of Life Care.

• Advocate for patients navigating healthcare systems.

• Collaborate with interdisciplinary teams to develop and implement patient care plans.

• Participate in interdisciplinary team meetings, Social Work meetings, and committee work to support patient care.
Requirements
• Master's degree in Social Work from an accredited institution.

• Current license to practice social work in the State of Michigan.
Nice-to-haves
• Proficiency in adjustment, loss, and bereavement issues.

• Experience supporting patients and families with chronic illness and end-of-life issues.

• Experience in medical and mental health social work.

• Current APHSW-C certification.
