This is a full time role that will work a Monday-Friday daytime schedule. Will support dialysis patients in a skilled nursing facility located at Westside Care Center, LLC, 349 Bidwell St, Manchester, CT 06040. Will work from home providing telehealth one day a week and see pts in the SNF 4 days a week. DaVita has an open position for a Social Worker who will be a vital member of each patient's core care team. Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them. In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. Requirements: Master's degree in Social Work (MSW) required with a specialization in clinical practice. Current Social Worker License in the state of Connecticut required Demonstrated knowledge of government and private insurance programs. Prefer Renal or Medial Social Work experience Basic computer skills in MS Word, Excel, PowerPoint, and Outlook as well as functional proficiency with DaVita specific applications within 60 days.
