Social Worker – Geriatric Care Manager (Remote)

We are hiring social workers to be dedicated care managers for older adults, covered by Medicare, so family can just be family again. We’re looking for social workers passionate, ideally with experience in palliative care, hospice, or geriatrics, who are passionate about making a difference in the lives of older adults and their families. This is a flexible, remote, 1099 opportunity with the path to grow into a leadership role on our team.
