Social Worker (Associate)-Primary Care, Mental Health-Full Tme-Olympia (Hybrid)
About the position
Responsibilities
ā¢ Confers and consults with supervisor regarding members progress and treatment and may consult with psychiatrist or other care team providers as well, if involved in members care.
ā¢ May provide appropriate support to members family, including explanation of treatment, instructions in how to support treatment and interventions to increase acceptance of and adherence to treatment, at members or surrogate decision makers request.
ā¢ Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment to include referral of the member and/or members family to external resources, as appropriate.
ā¢ Participates in departmental program development, implementation and evaluation.
ā¢ Reports safety concerns to mandated reporting agencies.
ā¢ Develops, implements, coordinates, and evaluates plan of care and/or clinical treatment programs for the diagnosis, treatment, and/or referral of Health Plan members with acute or chronic mental or addiction illness.
ā¢ Participates in staff conferences for care coordination such as selection, planning, and evaluation of plan of care and/or treatment programs.
ā¢ Instructs and counsels patients and their families regarding compliance with plan of care.
ā¢ Completes all documentation of the members treatment and progress in accord with Kaiser Permanente, state and NCQA regulations and in keeping with accepted community standards within scope of role and setting.
ā¢ Provides consultation to care teams, primary or specialty care providers and/or health educators on matters relating to mental health, addiction, health psychology, behavioral medicine, and/or long term care and advanced care planning.
Requirements
ā¢ Masters degree in Social Work (or Social Welfare from a clinical track), Clinical or Counseling Psychology, Marriage & Family Therapy, or related field required from an accredited college or university.
ā¢ Marriage and Family Therapist Associate License (Washington) OR Mental Health Counselor Associate License (Washington) OR Licensed Social Worker Associate-Independent Clinical (Washington).
ā¢ Prior completion of a practicum or internship in a clinical setting.
ā¢ Must be familiar with DSM-V TR as a means of diagnosis.
ā¢ Has experience in assessing a broad range of psychosocial needs, diagnosing and treating, psychiatric and/or addiction conditions.
ā¢ Full licensure expected within 3-4 years of hire.
ā¢ Excellent interpersonal and communication skills.
ā¢ Knowledge of social service agencies, state regulations, and professional board standards as is related to member treatment, patient rights, and member/patient confidentiality.
ā¢ Knowledge of Evidence-Based Practice, person-in-environment theory and/or psychotherapy research methods.
ā¢ Knowledge of the bio-psycho-social functions that contribute to mental health.
ā¢ Accuracy in assessing and/or diagnosing patients and developing effective treatment plans.
ā¢ Professional maturity and ethical integrity necessary for assuming professional responsibilities.
ā¢ Commitment to quality of service, teamwork, and participation in a highly interactive multidisciplinary clinic.
ā¢ Ability to complete multiple tasks/objectives in a timely manner.
ā¢ Must be able to work in a Labor/Management Partnership environment.
