. Develops strong, healthy relationships with patients and families, using motivational interviewing skills to establish team-based, comprehensive care plans
. Identifies barriers to care and identifies needed resources, supports patients and families in accessing needed care.
. Assesses for psych-social, emotional, mental, and physical health needs and refers patients and families to appropriate resources, provides follow-up support to help ensure needed care is accessible.
. Communicates effectively with patients, families, community resources and staff, acting as liaison between all patient care team members within the facility and in the community
. Functions independently and as part of team to provide high quality care for patients and families
. Manages time, multiple tasks, and caseload professionally, completing all work thoroughly, as assigned.
. Implements and follows company policies and procedures and adheres to HIPAA, OSHA, and other state and local regulations
. Documents all work in Epic using telephone and outreach encounters, flowsheets, and longitudinal plan of care
. Intermediate knowledge of Microsoft Outlook, Word, and Excel
. Tracks data necessary to demonstrate positive patient outcomes and adherence to insurance payer quality measures
. Participates actively in care team meetings, quality improvement work and other meetings as identified
. Completes home and community visits as needed, maintains valid Colorado driver's license and reliable vehicle for transportation
