Seasonal Community Engagement Specialist, Amador County

The Community Engagement Specialist will focus on outreach, scheduling appointments, and maintaining relationships with community partners to ensure members receive necessary care.ResponsibilitiesConduct in-person outreach attempts for members identified by health plan eligibility listsComplete Athena documentation for outreach attemptsSchedule intake appointments and assist with the completion of required formsAttend outreach events and professionally represent Zócalo Health to community members and partnersBuild and maintain relationships with community coalitions, CPI collaboratives, and other stakeholders to keep them engagedConvert leads to patients using internal documentation systemsDocument all outreach attempts and outcomes in Athena and the appropriate trackersAssist in sending patient mailers and making follow-up calls to leads generated from community events to enroll them in programmingRepresent Zócalo Health at community coalition meetings and collaborative eventsEnsure professional, consistent messaging and build trust with community stakeholdersSkillsMinimum of 1-3 years of experience in healthcare, community outreach, or a related fieldExperience in engaging with diverse communities and providing culturally sensitive communicationFluent in English and Spanish (verbal and written)Strong interpersonal and communication skills to build trust and connect with patients and community partnersDetail-oriented with excellent organizational skillsComfortable with technology and proficient in using electronic health records (Athena experience preferred)Ability to work a flexible schedule, including evenings and weekends, to attend eventsAccess to reliable transportation for in-person outreach and eventsBenefitsComprehensive benefits (medical/dental/vision)Generous home office stipendCompetitive compensationGenerous PTO policy including 6 paid holidays.Company OverviewNow serving patients in California, Texas, Washington, and Maryland (July 2025)!
