PHA Health is seeking a Registered Nurse or Licensed Practical Nurse operating as a case manager and patient advocate.
RN or LPN responsibilities are to work independently and with a team, providing support in all aspects of case management and patient advocacy. Responsibilities will include medical case management, medical advocacy, and prescription assistance. This will include researching and accessing the lowest cost provider or pharmacy to provide the service or medication, assisting the member and obtaining the script from the ordering physician, implementation and account setup, tracking and follow-up as needed. Applicant must be able to multi-task with attention to detail, possess strong customer service skills, ability to perform calculations, and a willingness to learn.
Job type: Full-Time, days, Monday through Friday
Compensation: $24-40/Hour DOE!
