RN Transition Navigator - Full-time - Utilization Management - Ochsner Medical Center - Baton Rouge
About the position Responsibilities • Facilitates communication between patients, caregivers, and healthcare providers. • Discusses alternative care options with patients and caregivers. • Assists with discharge planning needs. • Manages the planning process for patient movement through the healthcare system. • Prepares and executes post-discharge care plans. • Adapts communication and behavior to meet the needs of specific patient populations. • Performs other related duties as required. Requirements • Registered Nurse diploma required. • 2 years of related experience required. • Current registered nurse (RN) license in state of practice required. • Basic Life Support (BLS) certification required. Nice-to-haves • Associate or bachelor's degree in nursing preferred. • Prior experience as a Nurse Navigator or in Case Management preferred. • Certification in Case Management (CCM) preferred. • Health Care Management (CPHM) preferred.
