RN Case Manager - Transition of Care (Outpatient, Bilingual Spanish or Vietnamese) - Remote
**Overview:** Alignment Health is seeking a Remote RN Case Manager to support members transitioning from hospital or skilled nursing care. This role focuses on reducing readmissions, coordinating post-discharge care, and providing education and resources to patients and families-all conducted telephonically.
