What We Offer We offer a comprehensive benefits package, including: Competitive Salary: $73,000 - $78,000 per year, depending on experience Sign-on Bonus: Up to $7,500 Full Benefits Package: Choose from affordable PPO or HSA medical plans Paid Time Off: Enjoy time off to recharge and relax Free Healthcare Services: Up to $1,000 in free healthcare services per year, when enrolled in an Amedisys HSA medical plan Wellness Rewards: Up to $500 in rewards for completing wellness activities Mental Health Support: Up to five free counseling sessions per year through our Employee Assistance program 401(k) with Company Match: Plan for your future with a company-matched 401(k) Family Support: Infertility treatment coverage, adoption reimbursement, paid parental and family caregiver leave Fleet Vehicle Program: And mileage reimbursement (restrictions apply) Job Summary As a Remote RN Case Manager, you will be responsible for: Patient Assessments: Perform comprehensive patient assessments and develop individualized plans of care Care Coordination: Collaborate with the care team to coordinate patient care and services Patient Education: Teach patients and families about self-care techniques and promote health and independence Supervision: Supervise LPNs and HHAs to ensure high-quality patient care Documentation: Complete accurate and timely documentation Communication: Regularly communicate patient progress to the clinical manager and care team Staff Education: Plan and provide staff education to ensure ongoing learning and development On-Call Responsibilities: Participate in on-call responsibilities and provide services to patients and families as needed Clinical Development: Participate in clinical development and continuing education programs to stay current in your field Requirements To be successful as a Remote RN Case Manager with Amedisys, you will need: RN License: Current RN license in the state(s) you are assigned to work CPR Certification: Current CPR certification Clinical Experience: One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.* If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
